A social worker is a vital team member for all home health nurses and therapists. A
social worker's role is to tie the complete system of a patient's health and care

Medicare pays for patient home health services from a medical perspective, but
requires a systems perspective for the agency's services. In other words, what kind
of environment and support system does the patient have to ensure a successful
recovery? Nurses, therapists, and social workers are all required to document each
case based on the patient's diagnosis. Treatment is provided by nurses and
therapists but the entire system is treated by social workers.
A social worker's role is to act as an advocate for the patient after assessing his/her
emotional, social, cognitive, spiritual, safety, and functional abilities. Decision Point
